The Wellness Health Cards here are shared by our team member, M.B. She describes it as:
"This helped recognize my warning signs/symptoms when I had mania in 2008
and then depression in 2013. I was able to list what I can do to help myself and the third column was for what my supporters could do to help me. I shared these with my psychiatrist and he
really appreciated having these.
The last Wellness Card reflects more of what I currently may face.
Because I have done all of this work, I no longer have severe warning signs of mania or depression and I am really proud of myself, and how far I have come.My hopes is that these tools may help you on your journey to wellness and may you be able to live the life you’re meant to live.

I would like to share some tools with you that have helped me to be organized when I see my psychiatrist.
1. Agenda for Psychiatrist’s/Psychologist/Doctor/Etc. Appointment (this helps give a snapshot in time as to what I
am doing – what’s going right and what’s going wrong or is
challenging)
2. Doctor Appointment Preparation List (this helps if I ever have
Mental Health Medication Questions – I can use this form)
By having these documents, I am able to feel like I am in the
driver’s seat of my life. It helps me to be prepared for my appointments and it shows to the professionals that I am invested
in my wellness.

Selfcare is a gift we can give ourselves. Putting ourselves first shows that we have self love and self
compassion. Once we do this we are able to be there for others.
I have created a wellness plan that I call my “Formula for Staying Balanced”, so that I may live a healthy and happy life even with
the challenges that may come my way.
I have learned how to become an advocate for my needs and wants, and by having this plan, it has put me back in the drivers
seat of my life!!!
This package includes the plan I have set up for myself, and then you will get tools to create your own plan and be the advocate that you were meant to be!!!
